Confidentiality

In order to provide social work services to you, I will need to collect and record personal health information relevant to your current situation. This is a necessary part of the assessment and treatment process. Your information will be stored in a secure online practice management system.

All personal health information collected by me during the provision of social work services will remain confidential and secure except when:

• There are reasonable grounds to suspect abuse or neglect of a child under 16 years old
• There are reasonable grounds to suspect that you may cause serious harm to yourself or someone else
• It is subject to subpoena by a court
• Your prior approval has been obtained to disclose your information to another person or professional agency
